Your application can't be processed until we receive your required documents. You can upload them during the online application.
Always required
- Completed, signed application
- A copy of your child's birth certificate
- Proof of income — total monthly gross income for the household (about four current pay stubs, or tax documents such as a 1040 or W-2; an LES for military families)
- Proof of residence — a recent water bill, electric bill, or rental/lease agreement
If applicable
- Guardianship or custody documentation
- For a developmental disability or educational need: your child's developmental evaluation and proof of services, or an active IEP
- For a chronic health condition: documentation from your health-care provider
- For military families: an LES, or a VA disability award letter
By the first day of attendance
- Children's Medical Report (health assessment) signed by the doctor within the last 12 months, including hearing, vision, and dental screenings
- Up-to-date vaccination record
