I consent to the release and exchange of information with collaborating agencies (Harnett County Schools, DSS, etc.) to support my child’s participation. I give permission to administer screenings (vision, speech/language, hearing, developmental) at no cost to me. I understand a health assessment is required within 30 days of entry, or my child may lose placement. I understand the hours & attendance policy (6½-hour day, 5 days/week, at least 50% monthly attendance). I understand I am responsible for providing transportation. I understand I am responsible for any wraparound care fees (before/after school, holidays, summer). I agree to the parent involvement expectations (conferences, communication, transition activities).
